This brief form is useful for short term monitoring of behavior in classrooms.

Two or three weekly ratings are usually better than a single rating. Teachers should rate the child's behavior based on the last week's performance. This form can be reproduced without further permission but without modification. The best use of scores on this form is to compare a child's behavior over time. A scoring form is available based on norms published in a chapter by Barkley.
